An introduction to hospital discharge

Supporting someone during and after a hospital stay can feel overwhelming. There’s often a lot of information to take in, and it’s not always clear what your role is or what to expect. This course guides you step by step through the hospital discharge process, helping you feel informed, involved, and confident in planning a safe transition home.

What you will learn:

  • Discharge planning – what it is and how it supports a safe move from hospital to home
  • Your rights as a carer – how you should be involved and what support you can expect
  • Key hospital staff – who’s who and who to speak to during the discharge process
  • Confidentiality – what information can be shared and how privacy is managed
